Health & Wellness Review at home: $150 

This includes review of health history. Additional fees for other services.

Phone or Zoom Health & Wellness Review: Starting at $75 for up to a 30 minute Zoom Appointment.

Up to 45 minutes: $100

Up to 60 minutes: $125

 

Prescription Refill (non-narcotic): $30

*This is offered following a Health & Wellness Review so that medical history, allergies etc. can be discussed prior to refill being given.

 

Bloodwork Requisition: $30

*This is offered following a Health & Wellness Review so that medical history can be discussed prior to requisition being provided.

Ultrasound/CT/MRI Requisitions: $30 each

Specialist Referral: $50

Pregnancy Support Package:

Prenatal and Pospartum support packages including home visits.

• An Introductory Consultation

• 2 Prenatal Home Visits

• 24/7 On-Call phone or email support 

• Immediate Postpartum Support

• 2 Postpartum Check-Ins

• Phone/Text Support for 6 Weeks Post Birth

Postpartum Home Visit: 

Full physical assessment of mom and/or infant. Breastfeeding support.

Visits typically take 45-60 minutes.

Prenatal Classes: Starting at $75 

These can be individual or group, in home or over a video call, or any other location.

Postpartum Home Visiting Package: 

3 HOME visits on your choice of days. $450

MTO Physical- $175 (this includes a home visit to complete the physical)

 

APS (Attending Physician Statement) or APU (Attending Physician Update $90
